2008 Forms Status

We are pleased to announce that more than 75% of our membership has approved the attached 2008 forms package. This confident position allows CSIO to enter into our subsequent stages of French Translation and a technical testing and implementation review. Once these processes are complete, CSIO will announce the dates these individual comprehensive forms will be issued to the general membership for deployment. CSIO wishes to thank those members who participated in bringing these forms to their current state, and we look forward to our future deliverables together.

Commercial Vehicle Supplement

CSIO creates and maintains Standard forms on behalf of the Canadian P&C Insurance Industry. They are designed for paper and electronic use, as well as laser print on plain paper.

CSIO also creates forms to have data entered, this is to allow vendors time to develop and integrate the forms into their Broker Management Systems. CSIO encourages all brokers to contact their vendors to determine if, and when, the forms will be implemented for use in their systems. Additionally, brokers should contact their vendor to learn if, and how, a filled form can be attached directly to the vendor system.

The forms are available only to CSIO members, including brokers who are members of their provincial association. For those who are non members and wish to obtain access to CSIO Standard forms, click here for more information.

AVAILABLE FORM TYPES:

PDF (Portable Document Format) can be displayed on any Windows based PC using the FREE Adobe Acrobat Reader (Version 7). If you have an earlier version of Acrobat Reader, it will have to be updated to version 7.

In order to save your PDF document with the data entered, you will be required to purchase a PDF editor/writer product. You can visit the Adobe website, (www.adobe.com), to purchase an upgrade. However, searching the internet will provide many other available options to perform the same function at a reduced cost. One such product can be found at http://www.pdfill.com/download.html.

CSIO PDF Forms are not free products and may only be used by parties who are paid members of CSIO.

Unregistered use of CSIO PDF Forms is in violation of Canadian and international copyright laws. This agreement shall be governed by the laws of the Province of Ontario, Canada.

THESE FORMS ARE LICENSED "AS IS" AND WITHOUT WARRANTIES AS TO PERFORMANCE OR MERCHANTABILITY OR ANY OTHER WARRANTIES WHETHER EXPRESSED OR IMPLIED. Because of the various hardware and software environments into which CSIO Form software may be put, NO WARRANTY OF FITNESS FOR A PARTICULAR PURPOSE IS OFFERED.

You are specifically prohibited from distributing the PDF forms without prior written permission or from using the PDF forms for the benefit of other non-member parties other than your direct insurance broker customers and your direct insurance company customers who are CSIO members in good standing.
