CSIO creates and maintains Standard forms on behalf of the Canadian P&C Insurance Industry. They are designed for paper and electronic use, as well as laser print on plain paper.

CSIO also creates forms to have data entered, this is to allow vendors time to develop and integrate the forms into their Broker Management Systems. CSIO encourages all brokers to contact their vendors to determine if, and when, the forms will be implemented for use in their systems. Additionally, brokers should contact their vendor to learn if, and how, a filled form can be attached directly to the vendor system.

The forms are available only to CSIO members, including brokers who are members of their provincial association. For those who are non members and wish to obtain access to CSIO Standard forms, click here for more information.

AVAILABLE FORM TYPES:

PDF PDF (Portable Document Format) can be displayed and saved on any Windows based PC using the FREE Adobe Acrobat Reader.

For members doing their own development/implementation, WMF (Windows Metafile Format) is also available, upon request.
